GOT PERMANENT RESIDENCY. WHAT NEXT?

I just got my Bulgarian permanent residency. Could someone who is also a permanent resident here answer a few questions?

1) Can I get by with just the certificate of permanent residency, or am I required to get a photo ID?

2) If a photo ID is obligatory, that means I first have to get a personal identity number. Where does one go to get a personal identity number?

3) If a photo ID is obligatory, what office issues it?

4) Are there any websites where you can download application forms for either the citizen number or the photo ID?
