Claiming back Pension (AFP Modelo) and Finiquito de Trabajo

Hi everyone!

It has almost been three years since I left chile, to relocate to the Netherlands for study...

I worked in Santiago for 2.5 years (2011-2013), with contract and pension plan.
Before leaving I had planned to withdraw my pension, along with my finiquito de trabajo but unfortunately I was not able to due to miscommunication at the home end (UK) and I did not recieve my notarised documents in time back to complete the process before leaving.

My main question is, is there a way I can still claim it back? As far as I am aware I would have to work in Chile again/obtain a new cedula de identidad?

I still recieve cuartley updates on my pension.

In regards to the finiquito, I assume that is probably lost - but if you have any suggestions.

Thank you so much! I look forward to hearing your ideas and suggestions.
