Immigration hiccup avoidance question

My understanding is that we ought to come to Chile to initiate the visa process, which can take a year (or longer.) We understand that getting permanent visas is a bit fraught, and it's - or used to be, anyway - better to initiate in-country. If that has changed and it's possible to start the paperwork here in Japan, I'd like some advice/perspective/etc.
Our main concern is for the health insurance requirement for us as tourists as we wait to fulfill the permanent-visa requirements. International providers seem geared to provide only for a typical tourist stay of < 1 month, and are extremely expensive. I am wondering if it's possible to sidestep that issue once we're in-country - by possibly buying it from A Chilean provider?
