Retirees from the US, I Need Some Advice

My husband is retiring shortly and we will be in Cuenca this November. Those of you that qualify for medicare, have you signed up for it? Did any of you purchase international insurance? It's my understanding that in Cuenca, the new hospital there is going to be offering some type of plan that we may be able to buy into. Any pearls of wisdom be greatly appreciated!
