Two USA passport questions

First question. I am in the USA. I have a USA passport with a Ecuador residence visa. My passport expires in 4 of 2012. How far in advance of its expiration may I apply to renue it?

Second question: Same passport- is attaching the old passport (with the residence visa in the middle) to the new passport (using staples) the only way I can have both? I think I saw this solution in a gringo's pocket in Cuenca.

I have checked the State Department site and can find no answer to these questions. Also cant find a "search topics" function on this forum...though I seem to recall one in the past.
