Medical/EC gov requirements to bring dogs

I have spoken to the consulate in Vancouver a few times about this, but had a lot of difficulty understanding him as he rattled off answers that seemed to change each time the questions were asked.

We have moved to Ecuador about 6 months ago. One of the dogs we left behind isn't adjusting well and I'd like to ship her to Ecuador. I'm not sure if I can make the trip with her or if we'll have to ship her as cargo.

What are all of the requirements for her? does she just need a rabies shot or does she need all her shots? Is there a special form for the clean bill of health? Any other info anyone can provide me with would be greatly appreciated.

When we communicated with Etienne at the consulate in Vancouver he had so much info and he would rush through everything so fast and then what he said would seem to change. Ultimately we do need to get him to sign off on our dog for this but if we could get it as right as possible before he sees the paperwork it would be great.
