Entry to Ecuador as tourist.

I just returned home from 5 weeks in Ecuador. It was my first time in Ecuador and I really enjoyed it. It appears I could be there again December through January. This could stretch my stay past 90 days total. Does the 90-day tourist stamp start over January 1st or is it 90 days out of any 365-day period. How would you go about getting an additional 90 days if needed? I will be in the Manta area.
I just returned from the 5 weeks and loved the climate. This will let me check out some of the warmer months of the year.
Because of the tax situation in Colombia I may be deciding to spend (6 months + one day) out of the country, (Colombia) and Ecuador seems like a nice place to spend most of that time. I saw someone else posted a recent thread about tourist medical insurance in Ecuador. I will be watching that too to see what recommendations are posted there. Thanks for any advice.
