Health care; mosquitos, altitude...

Hello there, my wife and I are considering a move to Mexico or Ecuador, but doing research as of now. What are some of your experiences with the health care in Ecuador? Is navigating the system complicated?
Also am curious about mosquitos, altitude sickness, climate and a number of other things that we will think about as this journey of discovery continues.
