Dog with us to the US

We have ben in Guate for almost 3 months. We need to exit and want to take our small dog with us. We understand the airlines will only allow a pet to travel if there is a local vet checkup and a certificate by the Dept of Agriculture (or the equivalent). Does anyone recommend a vet in or near Antigua who can do this for a reasonable price?
