Authorised person to issue a TAJ card

Hello, dear Hungarian expats,

I need to change my TAJ card, but I want to know if my boyfriend can submit the application for a new card without me (I was already taking days off from work and i can't ask for that all the time, but he will have a day off anyway).

In the application form there is a field for a substitute authorised person. Do you know if I need a separate declaration where I declare his authorisation? And if yes - can it be written freely or I need a special paper, provided from the institution?

We heard that maybe we will need 2 witnesses. Is it necessary that these witnesses are in the office with my boyfriend or they can sign the papers at home and the he brings all of the papers by himself

We called and emailed the respective institution many times to ask them, but they do not answer.
