Health Care in Italy

I put this question out primarily to Americans who are currently retired in Italy. Are you comfortable being on the basic Italian health system? Or, have you opted to pay for private coverage in Italy as well? Do you still maintain health care in the US as back-up? Just wondering about the experiences or concerns that drove your decisions since I will have to do the same as a retiree in Italy sometime soon. (I have dual citizenship if that is relevant.)
