Copy of documentation needed for dual reisdency?

Hello,
Can anyone share with me a copy of the document that you received from the US Govt showing that your grandfather/grandmother was NOT a naturalized citizen before your father was born and that you submitted to the Italian embassy for consideration? Even better would be a copy of the entire submission you used to gain dual citizenship.

We are creating a comprehensive website that actually shows all documentation required. All of the ones we have researched have a list of required documents but not examples or the proper format etc.
