I just spent an hour or so on the phone with Medicare and Social Security, and have some additional information, should it be of interest.
For anyone who may be considering applying for Part B Medicare, it takes 30-90 days to process an application. Should you be traveling to the US and need care prior to your actual enrollment (meaning processing is complete), the cost of medical care WILL NOT be covered or reimbursed, even though you applied for Part B before your designated deadline.
In addition, if you need hospitalization after you've been approved, there is a $1340 deductible, plus a 20% co-pay.
To cancel Medicare, you simply need to download and fill out an online application, stating the date you want Part B terminated and reason you're terminating, and mail it. The post mark must appear, as it will serve as validation regarding the date of termination you listed.
The good news here - should you want to terminate Part B, it can take 30-90 days to process your application, however, by listing the termination date on your application, any premiums that were deducted from your SS payment will be reimbursed.
I hope I've written this clearly enough. I'll do my best to answer any additional questions.
