Confused by Italian rules for Italian citizen from USA and medical card

We both have Italian citizenship and passports... So... before we left the USA I was told that we had to stay for 3 months in Italy to get our Italian ID card and our medical card. So, we get to Pineto and rent a place (AIR BnB, by the way) for 4 months and have no idea what to do. Our landlords do not speak any English. I had written to them before we left the US that we wanted to get our EU credentials. The house we rented is new and apparently, it has no address. Not even a mailbox. We are in limbo and have no idea what to do. Can anyone help or suggest someone in the area who speaks English to help us? The clock is ticking and we are getting frustrated...
