Living Costs with Medical Concerns

Hi All. I apologize if this topic was covered elsewhere. We are a family of five (including one elderly), and considering a move to Philippines, Makati/BGC area in particular. I would appreciate any feedback to this three-part question.
1) For the BGC/Makati area, what range would a competent caregiver with nursing qualifications cost?

2) Considering caregiver costs and international school for two, would $5k USD monthly income be sufficient to live comfortably in Makati/BGC (assuming we have no rent expense).

3) I realize both places are expensive, and wondering if anyone can recommend an alternative with a good hospital and international school nearby. It doesn't have to be in Manila.
