13a visa

Hi all,
Looked in the forum but didn’t see what I’m looking for.
While living in the USA, I will soon be applying for my 13a visa. Been married 9+ years.
The RP consulate is in Chicago, about a 5 hour drive.
What am I in for doing this?
Can this be done with one trip to the RP consulate?
Approximate cost, and materials needed?
I plan on another vacation there before moving permanently.
ETA Philippines no sooner than next year.
Wife and daughter are dual citizens.
