Bringing cat to Portugal

Has anyone successfully brought a pet to Portugal? I'm being told different things from the consulate and the USDA's office. I understand I need a health certificate and a rabies certificate, both notarized by the USDA and approved by the consulate. Are their particular forms I need to use? Any advice would be greatly appreciated! Thanks!
