Autonomo for healthcare

I'm wondering about issues/problems with registering as an autonomo in order to have access to healthcare? I understand you need a work visa. I'm nearing retirement but might get my current US employer to contract with me for services which I could do remotely. I understand it's about 300 Euro per month but this would still be less expensive to us than the non lucrative visa route with the convenio especial which provides no prescription coverage. Do I have to make a profit? What if I make no income just pay the minimum social security contribution? If I close the business do I lose access to coverage? How difficult is it to get a work visa for someone self employed? Sorry if these have all been asked before. The information I find about the autonomo is how expensive it is and people don't have the money for the monthly social security payment. I'm sure I'm missing something, hopefully this group can enlighten me.
