Communicating with Spanish doctors

My wife and I are thinking of retiring to Spain but are concerned that our lack of Spanish-language fluency might be a problem when dealing with doctors and other health-care providers should the need arise. In your experience, is this a legitimate concern or do most of the medical people in Spain understand English well enough to communicate with us? Appreciate all input.
