travelling by bus with a suitcase

I'll be arriving to Managua and then will need to take another form of transport to the cities/towns where planes don't fly. I'll have a back pack and a large suitcase. Do bus drivers load luggage underneath or do I have to take it inside and keep it next to me paying for extra seat? Do you guys have any experiences with that? And also how safe it is to travel with large luggage on a local buses?
