Applying for EPS Sura. The way I am doing it.
First, in this forum we have been referring to EPS basic. There really isn't an category called EPS Basic. We have been using that phrase to differentiate from those who are buying Prepagada insurance, which is above and beyond EPS.
So, there is either EPS or there is Prepagada. This thread is about EPS Sura, and is my experience based on my application last month.
Last month I was covered by EPS Sanitas. But, i can only have one EPS, so EPS Sura would not accept my first application until I cancel EPS Sanitas.
As it turns out, it is not easy to cancel the EPS Sanitas insurance until I have had it one full year.
The one full year is now over, and I have cancelled EPS Sanitas. To do that required me to go to the Notaria and sign a one page document saying I no longer want to be covered by EPS Sanitas.
Now, I am making my second application to EPS Sura. The first step is to go online, www.epssura.com and click on: Afiliados.
Scroll to the bottom of the page and click on: Formulario de afiliación nueva / Traslado régimen contributivo
Fill out the form. Two points of interest are:
First, the form asks for Ingreso Base de Cotizacion-IBC. That is my base salary. I used the base salary I used when I qualified for my Permanent Visa.
Second, the form asks for my status. When I did the first application last month I clicked on 5 - Pensionado, but I found out later I should have clicked on 3 - Independiente.
The form is now approved for submission. I need to print off two copies and sign them and take them with me to the appointment.
To get the appointment it is supposed to be possible to do it online, but their web page is stuck and I can't get the appointment online, so I will have to go their office on Tuesday (Monday is a holiday) and use their computer to get an appointment.
I will update this post after my appointment.
