Health Insurance

I am not sure how much this topic has been already covered but bear with me.

It seems that there are 3 choices for health care in Ecuador.

1. Self insured or pay as you go.

2. Some kind of private plan that is similar to what we have in the US.

3. Whatever is available to permanent residents whether native born or pensioners.

1. and 2. are fairly easy to comprehend so what I am interested in is what any you expats are doing and also what opinions and info do the locals have on what the Ministry of Health has available and how it works?
