medical insurance on long-term travel visa

Hi All - I'm wondering if anyone has tried to purchase Ecuadorian medical insurance prior to becoming a legal resident. I will be entering Ecuador on a long term travel visa (6 months) and working on my resident visa during that time, but I'm trying to decide if I need to buy American emergency medical insurance. I undertsand from my Ecuadorian boyfriend that basic Dr visits arent terribly expensive there. Im more concerned about if i should need an MRI or something that would really cost a lot. Also, anyone that can recommend a good Ecuadorian med ins provider, would be greatly appreciated. Thanks!
