9-VI residency visa question

I am in the process of getting my papers together for the 9-VI visa and was wondering if there is anyone who has filed for residency lately that could clarify the following requirement:

Visa request submitted to the General Director of Immigration, in fact and legally sustained, signed by the interested party. The document will include the domicile of the interested party including address, telephone, fax , and personal e-mail—this is of the utmost importance as this is where advices regarding the request will be sent.

Where do I get this document from? Or does a lawyer have to write it up? Or can I just do it on my own?

Also they told me I need a legalized copy of my birth certificate, so should I just get an apostilled copy done in my home state? Or is there somewhere I could do that here in Quito?
