Living in a city in Ecuador, but having access to the ocean for scuba diving

If my wife is concerned about healthcare access, and I'm more concerned about having easy access to the ocean to go scuba diving, is there an optimal compromise? How difficult would it be to live closest to the most modern medical facilities in Ecuador, and still be within a reasonable drive to a place for scuba diving? Would such a compromise require owning a car?
