Chossing a doctor

I now have residency and will soon be visiting the nearest Agenzia Entrate to obtain my carta di sanitaria. Will I receive a list of doctors from which to choose a general practitioner (family doc)? If so, am I restricted to a doctor in my commune of residence or can I choose one in a nearby towns? Finally is there any help available to find an English speaking doctor?
