Denied A Visa?

I realize that extablishing residency in Italy for a non-EU (I'm American) individual is an intricate process and as I very much would like to retire in Trieste (I am learning Italian now) I am willing to go down that road. However, I do have a question. Does anyone know of anyone who has actually received an outright refusal of a visa? Thanks in advance for your input.
