Using US Healthcare Insurance in Italy

I will be retiring from the US Fed Govt and continuing my Blue Cross/Blue Shield Health Insurance as my husband and I intend to split our time between the US and Italy. We will be purchasing a holiday home in Perugia but will not be residents of Italy and will not be staying more than 89 days each trip, twice a year. We do not believe we would be using the Italian healthcare except in emergencies. Does anyone have experience using a US health insurance carrier in Italy? What issues have you found?
