Long term visa - Duration of stay

I am filling out my long-term visa application in preparation for my consulate appointment on Friday. For the number of entries (options: one, two, multiple), are they referring to how many trips out of Italy I would like to take, or is that a more specific question.

Also, for the duration of intended stay, it says a maximum of 365 days. I am hoping to stay for two years, so I am I going to need to leave Italy and come back? Should I put two years or 365 days?
