How to get S1 form

I'm trying to find information about how to get an S1 form. Sergios or anyone else who has done that, or tried to, can you please point me in a direction?

All I can find is that I have to go to the ASL where I am currently registered, so that's straight forward, but I'm trying to find out if I can apply by mail or email, or at least download an application form if there is one. I couldn't find any info on the Teramo province ASL website, and I'm in Germany at the moment. Is there a more official name for the form than S1? I haven't seen any other name on any of the EU sites that refer to the S1 form.

I am a little confused, because one website indicated that Italian pensioners; ie, people who receive Italian pensions, can get an S1 form. However, although I'm an Italian citizen, registered in the health care system, and retired, my pension doesn't come from Italy. How does that work, then??
