Preexisting Conditions and the Permesso di Soggiorni

My wife and I are researching the feasibility of retiring in Italy (from the United States). In order to get our Permesso di Soggiorno we need to show we have health insurance. My wife has serious preexisting conditions, and she would likely only be able to get a policy that excluded her preexisting conditions. Would this be a problem as far as getting our Permesso di Soggiorno? Our goal would be to get into the Sistema Sanitario Nazionale so that her preexisting conditions would be covered. Any help/advice would be greatly appreciated!
