Trying to understand the Elective Residency Visa

Hello,
my wife and I are working on getting our documentation in order right now. We plan on leaving for Italy June 2022, we just don't know if we have to file the ERV application separately or if one application is ok and if we have to file separately are we able to make our appointment at the same time? Since we plan on leaving in June 2022 how far in advance should we make our appointments, the embassy we have to go to is in Chicago, we don't know if that makes a difference.
