ERVisa

My husband and I are planning to apply for the ERV later this year at the Miami consolate. We've gotten answers to some of our questions from the consolate by email, but not many. Does anyone know:
1. Are ERV applications now only by mail, not in person?
2. How long before the consolate responds ?
3. Can /should we visit the consolate in advance WITHOUT an appointment to seek advice on completing the application?
4. Is proof of an airline ticket to IT required in the application?
5. If our application is not accepted do we get a chance to resubmit it?
Thank you for any advice you can provide.
