Italian Insurance

While Medicare does not cover out of USA medical bills, I have a Medicare HMO that cover up to 50K. I pay out of pocket and they reimburse me. I'm in good health and get my routine care when I'm in the states. If it is an emergency I know I can go to a hospital emergency department, But what is the process for basic care like needing an antibiotic for a sinus infection or chest congestion? It seems like I need to establish a general doctor who has agreed to treat me as needed. I do not qualify to Italy's Insurance program for foreigners.
