65 and over health insurance

If you are over 65, I would like to hear what you are doing about health insurance. Do you have private insurance? self-insure? And what does it mean to "keep medicare"? We have just turned 65 but are still employed, so we haven't yet looked at cost of supplemental care if we stay here. Do you purchase supplemental care in the US (in case you have to return someday) and also get private health insurance there? Or do you buy health insurance for Mexico and plan to pick up cost again someday if you return to US (and I am not even sure that is possible).
