Long-Stay Visitor Visa Extension (LSVVE) s

This sounds exactly what I am looking for.
Do we apply for this wll we are out of the Philippines, do we arrange when we arrive at the Manila airport OR do we enter on a 30 day tourist visa then arrange once we are in Manila?

Any advise is appreciated if you know of this visa
