Medical insurance dilemma

About ready to move to PI, but faced with insurance payments from Cigma which would be $500 a month, half of my pension of a 1,000. Being 68, I must be a liability, but now am dismayed. This is impossible! Surely, someone has a clue. Does Filipine marriage help at all? Please feel free to say anything at all. Thanks
