Tourist Visa set to expire

I (USA Citizen) have been living here in the Philippines for 5 and a half years now. I exit and return to the philippines every 3 years. I have several health issues where I am prohibited to travel.
I am wondering how I can proceed to stay in Philippines after my 3 years are up.
