fiancee -> spousal visa turnaround time

I currently reside in America and my fiancee in England. I hope to aquire a fiancee visa soon so I can move to London to live with him. Once we marry I an wondering how long it takes to change the fiancee visa to a spousal visa. Ive heard it can take up to six weeks. Is it possible to go to the IND office in person to have it done sooner?
Do I have to pay the visa fee again, or is it simply changed?
Do I have to submit all the documents (financial records, accomadation records, etc.) again, or do we just provide the marriage liscence?

Any answers are greatly appreciated!
