SSA and loss of benefits

Please forgive me. This is a question that obviously pertains to USA citizens. To carry on................
I received notice that my SSA benefits have been suspended. (FYI, I have received the benefits for about 2 years) OK, I say to myself herein lies an other of life's adventure........
I called SSA and was told that I hadn't responded to a letter they sent. Stuff like that happens, that is the letter never arrive here, but, for what it is worth, I have received other letters from the same SSA. Anyways I asked what were my alternatives and I was told to complete and return the form ssa 7161.
OK, simple enough, so I thought.
Upon closer inspection of said form ssa 7161; it is about receiving benefits for children or incapable adults!!!!!! Totally not my scenario! Never married and definitely no kids!!! No offense to anyone, por favor!
I will call SSA again and see what transpires.
So, I am asking, have any of you had this kind of experience?
